Application Checklist

1.  Income - two most recent year's W2's and 30 days of paystubs

2. Assets - two most recent checking/savings bank statements; mortgage statements

3. Taxes - most recent real estate tax bills

4. Insurance - declarations page showing current premium and time period covered

5. Identification - drivers license or other accepted state or federal-issued ID.
